This report is available online at truewheelers. org, and summarizes the findings of the classic 1977 Cross and Fisher study, A study of bicycle/motor-vehicle accidents: identification of problem types and countermeasure approaches (also available online -- first and second volumes at different locations -- volume 2 is in Google Books; volume 1 on a US Government Web site). Some of the data from these reports have been reparsed to account for rates of urban vs. rural cycling; also, while the data is robust, Cross's recommendations have been criticized: while he was an expert on data collection and research, he was not as much of one on bicycling -- see John Forester's book Bicycle Transportation for the critique.
