Visits may be cancelled as the result of illness, docket changes, or because a monitoring office just cannot get a qualified team assembled in time. In general, however, we prefer to cancel a visit rather than compromise on the quality or preparedness of the monitoring team. The information on Regional Directors meetings is provided at Tab 6. It excludes a listing for the Regional Directors meeting scheduled for October 11. That meeting has been postponed. Progress on the agenda items has been delayed as a result of staff time spent preparing this information. Annual Reports - This is the old report format introduced in 1979. It is a written report, often prepared by a single staff person. It may be based on a file review in the Regional Office, and does not necessarily involve an on-site visit. Independent Evaluation -- This is a formal on-site review of a program's effectiveness as required by statute [42 USC 2996(F)(d)]. Full scale independent evaluations involve an on-site review by a team of independent consultants and staff, an inspection of files and records, and generally an assessment of recipient funds management.
